About Us A Commitment To Quality
Mission Statement
Our mission at Cross City Nursing and Rehabilitation is dedicated to helping our guests not only recover, but live better lives by providing the highest level of medical treatment and patient care possible. We treat our patients with the utmost sensitivity and respect, ensuring their complete comfort during their stay. Our facility thrives on our commitment to individualized, top-quality care. Each day, we seek to provide a family-like experience, with a warm, yet professional atmosphere for each of our guests. This commitment is what makes us distinguished within the peaceful community where we are located.
Our Approach
We understand the challenges one faces when choosing a rehabilitation or long-term care facility for a loved one. Cross City belongs to a family of facilities dedicated to providing the highest level of care possible for both our short-term friends, and long-term family. We implement a unique method to providing care, one that focuses on nurturing the overall mental and physical wellness of our residents, not just treating medical issues. Our state-of-the-art facilities combine cutting edge technology with a warm, friendly atmosphere. Aside from our top notch, round-the-clock medical care, guests can look forward to community outings, fun activities, luxurious rooms, and hands-on, delicate care.
For our long-term and short-term residents, we offer an individualized plan of care and services tailored to meet specialized needs. For our short-term rehabilitation patients, we focus on strengthening our patients so that they can return home as soon as possible. Family education, training, and care conferences are also available to address longer-term goals and discharge planning. In order to ensure a smooth transition after discharge, home evaluations are completed for most rehab patients returning home within about 1-2 weeks.
For our long-term patients, we do our utmost to ensure that our patients receive the best possible care. Our experienced staff maintains constant communication with our patient's medical team as well as their family members. At Cross City, you can rest easy knowing that your family member will receive warm, compassionate care, making his or her stay with us both comfortable and enjoyable.
